It is a structured conversation, not a test, and it will feel unexpectedly like being interviewed about your entire life. Childhood, school, work history, relationships, money, driving, substance use, sleep, mood.\n\nSeveral questions will feel oddly specific. Do you lose things you need regularly. Do you start conversations at inappropriate moments. Do you find it difficult to wait your turn. These map directly onto formal diagnostic criteria, and the clinician is establishing not just whether each thing happens but how often, since when, in which settings and what it has cost you.\n\n### Rating scales are not the deciding factor\n\nYou will almost certainly fill in questionnaires, either beforehand or on the day. These are useful, standardised and easy to score, and they are not what determines the outcome. They are one input into a clinical judgement.\n\nThis surprises people who expect something more objective. There is no blood test, no brain scan and no computer task that diagnoses ADHD. Computerised attention tests exist and some clinics use them, but they are supplementary rather than diagnostic. This is not the clinician trying to talk you out of ADHD.\n\n## Getting Assessed in the UK\n\nThis is where the honest version diverges sharply from the marketing version.\n\n### The NHS route\n\nReferral starts with your GP. The assessment is free, the quality is generally good, and the wait is the problem.\n\nThe scale is now documented officially. There is no ADHD-specific waiting time standard and no national reporting of adult waiting times, and the Taskforce specifically flagged that high-quality data collection needs to be mandated for all providers. Waits vary wildly by area, and reports of multi-year waits are common. Any website quoting you a precise national average is estimating, usually from crowd-sourced patient reports, and often while selling something.\n\n### Right to Choose\n\nIf you are in England, this route is easy to miss and can save you years. The [NHS Choice Framework](https://www.gov.uk/government/publications/the-nhs-choice-framework/the-nhs-choice-framework-what-choices-are-available-to-me-in-the-nhs) gives patients a legal right to choose their provider at the point of referral for a first outpatient appointment, including providers outside the NHS that hold an NHS contract for the service. It applies to mental health as well as physical health, and the assessment remains free.\n\nThe limitations are real. The framework applies in England only, so it is not available in Scotland, Wales or Northern Ireland. The right applies at the point of referral, which means your GP has to make it and you cannot self-refer. Individual providers then set their own eligibility rules on top of the scheme, and many adult ADHD services will only accept patients aged 18 or over, so check the provider's criteria before you ask for the referral. And it is not a shortcut to nothing: the popular providers now carry substantial waiting lists of their own, and demand has grown faster than capacity there too.\n\n### Going private, and the catch nobody mentions\n\nPrivate assessment is faster. Advertised prices vary far more than most people expect, commonly running from around £400 to £1,500 for the assessment itself, with a median somewhere near £950. Assessments led by a nurse prescriber sit at the lower end and consultant psychiatrist led assessments at the upper, and packages bundling in titration can reach £2,000 to £2,900. Appointments can often be arranged within weeks. Compare several providers before committing, because the spread is enormous for broadly similar work.\n\nHere is the part the clinic websites tend to underplay. Getting diagnosed privately does not guarantee you can get medication on the NHS afterwards. Ongoing prescribing usually depends on a shared care agreement, where your GP takes over prescribing after a specialist has stabilised your dose. The [BMA is unambiguous](https://www.bma.org.uk/advice-and-support/gp-practices/prescribing/prescribing-in-general-practice/principles-for-shared-care-prescribing) that this is \"non-core voluntary activity that can be declined by the GP practice for any reason\". Published figures put private ADHD prescriptions at roughly £30 to £180 a month depending on the drug, the dose and whether the prescriber's fee is bundled in. On top of that, providers commonly charge a fee for issuing each prescription, often around £25, plus an annual review fee in the region of £180. Budget for the recurring cost rather than the headline assessment price, because that is the number you will keep paying.\n\nSome practices accept shared care readily, others operate blanket policies against it, and the position can differ between two surgeries in the same town. Ask your own practice what they do before you spend anything, not after.\n\n::inlineCta\n::\n\n## Getting Assessed in the US\n\nThe structure of the problem is different. Access is generally faster, and the obstacle is cost and coverage rather than queueing.\n\n### Who can diagnose you\n\nA wider range of professionals can diagnose adult ADHD in the US than many people expect: psychiatrists, clinical psychologists, neurologists, primary care physicians, nurse practitioners and clinical social workers, depending on state scope of practice rules. CHADD's guidance is that in many cases the clinician's knowledge of adult ADHD matters more than which professional degree they hold, which is sensible advice and a good filter when you are choosing.\n\nThat matters because your primary care physician may be able to diagnose and treat you directly, which is considerably cheaper and faster than a full neuropsychological evaluation. It is a reasonable first conversation to have.\n\n### What it costs\n\nCosts vary hugely, and depend on what you are really buying. A diagnostic interview with a psychiatrist or physician sits at the lower end. A full neuropsychological or psychoeducational battery, which involves hours of cognitive testing, sits at the top, and can run into four figures.\n\nWhether insurance covers it depends on your plan, whether the provider is in network, and how the evaluation is coded. Diagnostic interviews are more commonly covered than extended psychological testing, which is sometimes classed as educational rather than medical. Two practical steps save money and frustration: ask the provider which billing codes they will use, then ask your insurer whether those specific codes are covered and what your responsibility will be. Get the answer before you book.\n\nTelehealth ADHD services have expanded access considerably and are often cheaper, but quality is variable and some carry restrictions on prescribing controlled substances depending on your state and the prevailing federal rules. Check whether a service can actually prescribe where you live before paying.\n\n## After a Diagnosis: Titration and Supply\n\nA diagnosis is the beginning of a process rather than the end of one.\n\nIf you start medication, expect titration: beginning at a low dose and adjusting upwards over weeks with monitoring of blood pressure, heart rate, sleep, appetite and effect. Getting the right medication at the right dose commonly takes a few months and some trial and error. That is not the clinician dismissing you, and it is not a verdict that your struggles are imaginary. Something is still causing them.\n\nAsk two questions before you leave: what did they think was going on instead, and what would they suggest you do about it. A negative ADHD assessment that identifies untreated anxiety, a sleep disorder or burnout is a useful outcome, not a wasted one.\n\nIf you genuinely believe the assessment was inadequate, for example if it ran to twenty minutes with no childhood history taken, seeking a second opinion is reasonable. If it was thorough and the answer was still no, repeatedly reassessing until someone agrees with you is rarely the path to feeling better.\n\n## Frequently Asked Questions\n\n### How long does an adult ADHD assessment take?\n\nThe appointment itself typically runs between one and three hours, sometimes divided across two sessions, plus questionnaires completed beforehand. The wait to get that appointment is the variable part: private assessments can often be arranged within weeks, whereas NHS waits in the UK run from many months to several years depending on where you live, and there is no official national reporting of adult waiting times to pin that down more precisely.\n\n### Do I need my parents involved to get diagnosed as an adult?\n\nIt helps considerably but it is not always essential. Assessments rely on evidence that symptoms were present before the age of 12, and someone who knew you as a child is the most direct source of that.
